Jane’s Care provides NDIS supports for people with a wide range of disabilities and complex support needs, with particular expertise in psychosocial disability, including co-occurring physical disability and mobility-related support requirements. We work closely with participants and their stakeholders to create a stable start and deliver consistent, goal-aligned supports, backed by clear reporting and evidence that strengthens planning meetings and plan reviews.
The foundation of Jane’s Care is simple: Clear processes, strong governance and stable environments where participants can safely build capacity, routine and community connection.
A key point of difference in our model is how we approach home. Wherever possible, we prioritise single-occupancy living to support dignity, privacy and emotional safety, while still maintaining shared support ratios through apartment-style complexes or communal properties. This allows participants to have a place that is truly their own, without losing the benefits of shared staffing, community connection or cost efficiency. The result is greater choice, reduced friction and living arrangements that are both sustainable and genuinely participant-centred.
At Jane’s Care, our approach is grounded in consistency, clarity and genuine participant choice. We work to create stable environments where people feel heard, supported and empowered to progress toward their goals, even when things get complex.
Jane’s Care operates from North Perth as a central base, allowing us to extend supports efficiently across the northern, central, eastern and southern corridors of the metropolitan area. This central positioning enables responsive service delivery, reduced travel barriers and consistent care, while still ensuring our teams develop strong, local understanding of the communities they support.
We take a practical, participant-first approach to support delivery. Everything begins with what matters most to the person we are supporting, ensuring care is shaped around real goals and real lives.
We start with what matters most:
From there, we build supports that actively reduce barriers to community, social and economic participation, helping people engage more fully in the life they choose.
Clarity is built into everything we do. Information is shared in ways that are useful, timely and easy to act on.
We keep information clear and usable:
As needs evolve, so do our supports. We adapt with purpose, not disruption to ensure:
The result is a service that is steady, responsive, and always anchored in choice and control.
We work with participants whose needs are complex, layered and often misunderstood. Our focus is on individuals who benefit from structured, consistent support and a service model that can hold complexity without losing sight of the person.
Participants we support may experience:
Our role is to provide stability where it has been missing, and to build environments where people can feel safe, supported and capable of moving forward.
Team structure
Our service is supported by a multidisciplinary team designed to provide stability, accountability and continuity across every layer of support. Each function plays a clear role in ensuring participants experience consistent care, responsive coordination and a workforce that is prepared for complexity.
Our structure includes:
