When doing a home exploration, what exactly are you hoping to find? At Jane’s Care we provide just that, a home. We want our participants to feel comfortable, safe and empowered. Our team is trained to encourage community integration and inclusion in every house.

We understand that the number one gap for people with disabilities in Australia, is social inclusion and accessibility. We pride ourselves in our ability to create healthy, long term connections. We uphold our values and this can be seen throughout our models of care.

Currently, all of our homes remain within 30 minutes of Perth CBD. We work with single and shared tenancies, providing participants with affordable rates and quality supports.

Our team works hand in hand with therapists and staff to ensure participants are receiving the best care designed specifically for them.

Each participant is allocated to a Case Manager, this is an unique service Jane’s Care offers. (Not charged additionally). We understand the frustration experienced by participants, families and external providers when dealing with NDIS and transitions. By having this add on support, our team is able to smooth out all communication and act as a second support between coordinators, families and staff.

Short Term Accommodation (STA) / Medium Term Accommodation (MTA)

Current accommodation not meeting your needs? Let us look after you. We offer Short Term Accommodation for those requiring some respite or urgent hospital discharges.

For those in the market, waiting for a suitable home, we also offer Medium Term Accommodation with the same care and support as our full timers!

Positive Behaviour Support (PBS)

Jane’s Care’s accredited Positive Behaviour Support Practitioners will work closely with you, your family, stakeholders, and support workers to develop individual intervention plans and strategies that allow you to live your, your way.

We can help with assisting in developing communication and social skills and building your mental health, flexibility, and resilience, preparing you for school, work, or other social activities.

Your NDIS Positive Behaviour Support Practitioner will work closely with your other service providers, such as a Support Service Provider, Occupational Therapists, Speech Pathologists and Physiotherapist, for a holistic, person-centred, and flexible service focused on your personal needs.

Life Skills & Capacity Building

All of our team on ground are trained and hand picked based on their ability to encourage participants to reach their potential. Our team works directly with the participant’s choice of Allied Health professionals to create and implement individualised programs to improve quality of life. From Personal Support to Community Access, our passion is driven by helping participants to be happy, feel connected and find a purpose.
