Home care is an integral part of the care continuum with focus on desire, needs, goals and abilities.

Jane’s Care facilitates the prevention of and/or the delay of admission to a long term care facility and helps reduce the frequency or likelihood of readmission.

Quality service includes working cooperatively with clients as well as family. We also need to work hand in hand with advocates, GP’s, specialists, volunteers, NDIS, Disability Services WA, Department of Health (Aged Care), DVA, ICWA, pirate health insurers and as well as a range of audited and approve private providers.
